Litespeed & A2Billing 404 Error

Hello:

am trying to get http://www.asterisk2billing.org/ working under Litespeed but whenever I post any forms it always returns a 404 error. The URL on the form post that is being generated looks like: http://sitename/admin/Public/id="myForm" and the HTML code:

<form action="id="myForm"" method="post" name="myForm">
</form>

This all appears to work fine under Apache so presume something has been mis-configured in LSWS ?

There should be a rewrite rule in .htaccess to rewrite the URL to a PHP script.
