Zero Length Content with AuthType Digest

I've been trying to setup v 2.2.6 to do digest authentication (instead of basic). I have tested with Firefox, Opera and IE, none can authenticate with the server and receive data.

Allow Override contains Auth for the VHost.

Contents of .htaccess file:
AuthType Digest
AuthName "Restricted Area"
AuthUserFile /var/www/localvhost/config/digestusers
Require valid-user

There is nothing in the DEBUG log that suggests a problem; and I have tried clearing browser caches, etc...

Firefox: 0-length data, index.php downloads an empty file (MIME also broken)
Opera: 0-length data, index.php MIME works
IE7: fails digest authentication

Digest authentication is not supported yet.
